Tough new restrictions on the availability of Alzheimer's drug treatments will deny them to thousands of people across the country in the early and late stages of the disease. As this newsletter goes to press, the National Institute for Health and Clinical Excellence NICE ; has published its final decision on offering the only drugs licensed to treat Alzheimer's disease on the NHS. The NICE guidance would mean that Ebixa which can help people in the later stages of Alzheimer's will not be available on the NHS. Furthermore, the drugs Aricept, Exelon and Reminly would be withheld from people in the early stages of Alzheimer's. The decision could affect new patients from as early as July. People currently on the drug treatments will continue to receive them. NICE have revised their original plan for a total ban on prescription of Alzheimer's drugs, but we believe their philosophy is still seriously flawed. Withholding helpful drugs from people in the early stages of Alzheimer's means they will be denied treatment until their condition has deteriorated into "confusion and fear" and drugs are considered to represent good value for money. We believe this is unethical. As a member of Action on Alzheimer's Drugs an alliance of over 30 organisations demanding fair access to Alzheimer's drugs we are continuing to express our concern and yours. Since elevated glucose contributes to most of the complications of diabetes, it is important to use diet, exercise, and aggressive medication regimes to obtain as tight a control of glucose as possible. The American Diabetes Association ADA ; has published goals for glucose control, and recently, the American Association of Clinical Endocrinologists AACE ; has modified those goals by adding recommendations for postprandial glucose measurement and control, and by lowering the goal for HbA1c. The full set of both ADA and AACE recommendations are shown below in Table 1, for example, drug reminyl treat. Reminyl used to treat whatAted solution within the rubric of Moldovan law, not a right to be autonomous or to secede simply because you disagree with fiscal policy. c. No Other Solution Litskai argued that the people who have come to power in Chisinau in 1990 aimed the Moldovan State's mechanisms against Transnistria and that to defend themselves Transnistrians had to create a State in order to respond. 169 The unitary Moldovan state would not provide the guarantees that the Transnistrians needed and, as such, separation was sought, although, as Smirnov, Litskai and Marakutsa each emphasized, some form of federation or confederation may now be possible. So, if the TMR is now willing to consider federation, is it accurate to say there is no other solution? We should consider their argument here in the terms that it would need to be made to support a claim for external self-determination. In any case, as was discussed in Part III.B, above, the TMR's argument seems to actually be for full sovereignty, though approached obliquely. ; Their claim that there is no other solution but for secession is not persuasive. First of all, the actual history of Moldova since the end of the 1992 War shows that minority rights have been respected to a greater extent than feared. Although Moldova does not have a pristine record, if such a human rights record was enough to lead to a right of secession, the world would be rife with secessionist conflicts. This conflict has been frozen not so much because there are no other options under domestic and international law besides secession, but because the separatists gained by making the conflict seem intractable. As one commentator put it, "Russophone leaders [in Transnistria] used ethnic outbidding to exacerbate mass hostility and the security dilemma in order to preserve and increase their power."170 Head of TMR internal security Vladimir Antufeyev, for example, "runs a number of social organizations and newspapers that inflate the nature of the Moldovan threat to Transnistria.171 Furthermore, "[s]eparatist violence occurred because Russophone elites had much to gain, especially increased power and career opportunities for themselves, by promoting it."172 The problem may not only be in Transnistria. A NEW EVOKED POTENTIAL COMPONENT DURING SLEEP IN CHILDREN Colrain IM Center for Health Sciences, SRI International, Menlo Park, CA, USA, Psychology, University of Melbourne, Melbourne, VIC, Australia Introduction : It has been known for nearly 70 years that K-complexes KC ; and vertex sharp waves VSW ; can be elicited by stimuli presented during NREM sleep in adults. We sought to determine the impact of Tanner stage on phasic events elicited during stage 2 sleep in children. Methods : Data were collected from 12 pairs of same sex twins and one singleton subject aged between 8 and 17 years. One twin from each pair was selected for the present analysis. Sufficient responses were obtained from 12 subjects 6 boys ; , with 4 in T1, 3 in T2, and 5 in T3 T4, which were combined for analysis. 64 channel EEG data were collected from a single night can using Neuroscan Synamp2 hardware and Scan 4.3 software. Responses were collected to auditory 80db 50 ms ; tones presented binaurally via inert earphones. Data were scored for sleep stage and then for the type of phasic event produced by each stimulus within stage 2 sleep. Results : Traditional KC responses were elicited to 4812% of stimuli. In addition however, a previously undescribed evoked response was elicited to 168% of stimuli. It consisted of a positive-negative-positive complex, with a very large negative component -20026 V in T1, -13924 V in T2 and -12637 V in T3 4 ; approximately 350 ms. The complex lasted approximately 700 ms. The negative component displayed significant site p .001 ; and site by Tanner stage interaction p .001 ; , being largest at Cz, but having a broader scalp topography in T1 and T2 than T3 4. Conclusion : Children can readily elicit K-complexes, but also produce an additional delta frequency response with a vertex maximum and a much shorter latency. This component would appear to have features of both a VSW scalp distribution and latency ; and a KC period reflecting delta generation ; . Support optional ; : HL058585. These conditions should be treated with medication and adequately controlled; having nash or elevated liver enzymes should not lead people to avoid treating these other conditions and salmeterol. Ndekha A. Strengthening community participation in schistosomiasis control: lessons from the Guruve District Zimbabwe ; schistosomiasis control programme using Phytolacca dodecandra a plant molluscicide ; . Copenhagen: The Royal Danish School of Pharmacy, Department of Social Pharmacy 2001. 96.3% ; were bacteremic, 74.1% had septic shock, 59.3% had ARDS, and 44.4% required continuous renal replacement therapy. The patients were severely ill, with a median APACHE II score of 27 and a median of three organ dysfunctions. Not surprisingly, almost half of the patients 48.1% ; died. This is similar to the predicted mortality using the APACHE II model, which was 46.4%, as well as other published data17, 18 on outcome of community-acquired bacteremia in critically ill patients. For example, in a large, prospective, multicenter study of community-acquired bacteremia in ICU patients by Valles and coworkers, 18 the crude mortality rate was 41.5%. Factors in that study that were associated with mortality included the APACHE II score, septic shock, and inappropriate antibiotic treatment. With regard to the clinical features and patient demographics, our study population representing a subset of critically ill patients with melioidosis was generally similar to previously published series4, 7, 19 of all patients with melioidosis. The patients were usually middle aged, had underlying medical comorbidities, and diabetes mellitus was the major predisposing factor. Nevertheless, there were two major differences that are worth highlighting. Firstly, we were impressed by the overwhelming male preponderance in our series male to female ratio of 26: 1 ; . This is much higher than the ratios published in other studies2, 7, 19 range, 3.2: 1 to 4.5: 1 ; . Also, when we examined our data for all patients who received a diagnosis of melioidosis during the study period, we found a male to female ratio of only 6: 1. Why this discrepancy occurred is not apparent. It is interesting to note, however, that in a large epidemiologic study20 of sepsis in the United States, men were more likely to have sepsis than women mean annual relative risk, 1.28; 95% CI, 1.24 to 1.32 ; . The reason for this is unclear, although preliminary research by Hubacek and coworkers21 suggest that this gender-specific predisposition to sepsis may be genetic. They found that the presence of certain lipopolysaccharide binding protein genotypes with a less common Gly98 allele was associated with sepsis in males, a finding not repeated in females. Secondly, we found that our ICU population had a higher incidence of bacteremia 96.3% ; compared to the incidence of bacteremia in all patients with melioidosis. For example, in a prospective study over 10 years, Currie et al2 found that 46% of their cohort of all-comers with melioidosis was bacteremic. An epidemiologic study7 from Singapore of similar design found the rate of bacteremia to be 59.7%. In both these studies, bacteremia was associated with a higher mortality rate. As patients in the ICU would generally have a greater severity of illness and mortality rate, a higher incidence of bacteremia was not unexpected. It was noteworthy that patients who presented with pneumonia had a mortality rate of 45.4%. This compared favorably with two previous studies8, 9 of communityacquired pneumonia in the ICU from Singapore, both reporting a mortality of 100% among patients with melioidosis. We found it difficult to compare these previous studies both performed in the early 1990s ; with the present cohort, as the periods under study were different, and we have no comparable information with regard to severity of illness. We initially postulated that our better. A Heinz, M Smolka, D Mller, J Wrase In behavioral genetics, additive gene-gene effects are often assumed but difficult to test due to the small explained behavioral variance. Processing of unpleasant stimuli in the amygdalae has been associated with a functional polymorphism val158-met ; in the catechol-O-methyltransferase COMT ; gene and independently with a functional polymorphism in the regulatory region of the serotonin transporter 5-HTT ; gene. 5-HTT function may also be affected by a recently detected A G exchange in the long allele insertion ; of the 5-HTT regulatory region. We tested whether these three genotype effects are additive and explain a substantial part of the variance in amygdala activation elicited by unpleasant cues. Genotype effects on central processing of standardized affective visual stimuli unpleasant and neutral ; were assessed in 48 healthy men with functional magnetic resonance imaging. In individuals with more COMT met158 alleles and with more s or lA alleles of the 5-HTT regulatory region, aversive stimuli elicited greater neuronal activity in the bilateral amygdala and hippocampus. COMT val158-met and 5-HTT genotype were additively associated with increased processing of aversive stimuli in the amygdala, indicating that functional brain imaging may be used to assess the interaction of multiple genotypes on neuronal activation. Group A and 27% in Group B. These figures suggest a difference in incidence of PONV between children receiving opioids and those not receiving opioids however the numbers are too small to permit statistical analysis. Discussion Otoplasty is an elective cosmetic procedure performed on healthy children and should, therefore, be associated with minimal morbidity. Unfortunately this is not the case, severe pain and a high incidence of PONV make the child's first postoperative day particularly distressing.1-24 Opioid analgesics compound the problem of PONV, 12 cause excessive sedation and delay early resumption of oral fluids. Alternative methods of providing postoperative pain relief are regional analgesia8-9 and local anaesthetic infiltration.1011 The success of regional nerve blockade in the management of pain post otoplasty has been demonstrated.1 Local anaesthetic infiltration of the pinna using lidocaine 1% with adrenaline is performed routinely by the surgeons in our hospital in order to reduce bleeding at the surgical site during otoplasty. Local anaesthetic infiltration should, in addition, provide postoperative analgesia. In our hospital regional nerve blockade using bupivacaine 0.5% is performed routinely by the anaesthetist. The aim of this study was to compare two analgesic techniques already in use in our hospital with regard to quality and duration of analgesia, opioid requirements and incidence of PONV. In our study we found that both techniques provided excellent and comparable analgesia. Mean duration of analgesia was 8.6 1.1-24 ; hr in the group that received lidocaine 1% infiltration Group A ; . Mean duration of analgesia was 10.5 1.3-24 ; hr in the regional nerve blockade group Group B ; . The degree of pain control achieved by both regional nerve blockade and local anaesthetic infiltration resulted in a low requirement for opioids in this study, 24% of children in Group A infiltration ; and 14% in Group B nerve block ; . Interestingly, 24% of children in Group A and 27% of children in Group B required no supplemental analgesia postoperatively, analgesia extended beyond 24 hr, for example, ebixa. Based on record review, interview and observation the license failed to have accurate and up to date client records for one of two clients #1 ; reviewed. The findings include: Client #1 was admitted June 23, 2003.
