I have bought a menopause supplement containing isoflavone phytoestrogen, red clover and black cohosh. For the isoflavone levels of healthy subjects who can eliminate the phytochemicals efficiently via the kidneys, but it may affect substantially the levels of patients who tend to accumulate the isoflavones because of lack of renal function. The time of the blood collections and the ingestion of food on collection day did not seem to affect the baseline concentrations in the ESRD population, since the results were comparable in patients studied in the early morning while fasting or around noontime after intake of food. This suggests that as long as the diet is isoflavone-free or isoflavone-poor, baseline measurements in the ESRD patients can be obtained at the time of dialysis treatment, regardless of food intake. The five subjects that had detectable levels in four separately tested baseline samples showed considerable variation of the isoflavone concentration from week to week. This is consistent with observations in healthy subjects 17 ; , and it suggests that measurement of genistein and daidzein in single random samples in ESRD patients may not reflect with sufficient accuracy the prevailing isoflavone levels. Pooling of sample from at least three weekly drawings may be a reasonable compromise between the cost of the test and the need for representative values for a given patient. Additionally, it is noted that the fluctuations over time of the baseline concentrations of genistein and daidzein are not always consensual. A possible explanation of this finding is that the isoflavones have distinct degradation pathways that depend, to a good extent, on the intestinal flora 21 ; . Thus, variations of the intestinal flora over time may favor the degradation of one phytochemical over the other, in turn affecting their blood levels. Statistical comparison limited to the measurable baseline levels of genistein and daidzein was not possible due to the small number of such values in the control group. However, inspection of the available data Figure 2 ; shows a clear tendency for the ESRD patients to have higher genistein and. There were significant differences among population, environment, and their interaction for individual isoflavone content Table 3 ; . Population effects were not significant for total isoflavone content, but environment and population environment interaction were significant. Daidzein, glycitein, and genistein content of Pop. 3 were significantly different from the other populations Table 4 ; . Soy isoflavone and cancer

Petitioner knew what the petitioner was doing on the day of the crimes but that the trial court ruled the petitioner's doctors could not testify about a possible diminished capacity defense. He said that he decided not to call Dr. Fider to testify at the petitioner's trial. He did not remember the petitioner telling him that a juror had seen the petitioner in shackles outside the courtroom, and he stated that if the petitioner had told him about it, he would have brought it to the court's attention. He said that no doctors testified at the petitioner's sentencing hearing because the doctors had testified in a pretrial hearing and the hearing was part of the record. He said that at the sentencing hearing, the trial court made it clear that it did not need anymore proof regarding the petitioner's mental capacity. On cross-examination, the petitioner's trial attorney testified that he argued the petitioner's diminished mental capacity as a mitigating factor at the sentencing hearing. He stated that he did not tell the petitioner that the petitioner would receive a twelve-year sentence. In denying the petition, the trial court ruled that there was nothing the petitioner's trial attorney could have done to correct the dose of the petitioner's medication. Moreover, the trial court noted that during the petitioner's psychiatric evaluation, the petitioner did not show he was having any problems with his medicine. The trial court ruled that the petitioner's attorney adequately prepared him to testify. Regarding the attorney's failure to argue that the petitioner's mental condition mitigated his sentences, the trial court stated that "the Court knew of his mental health questions prior to that since that was brought in by reference into the sentencing hearing." Regarding a juror's seeing him in shackles, the trial court stated that there was no evidence the event actually occurred, demonstrating that the trial court did not believe the petitioner's testimony. The trial court denied the petition for post-conviction relief, and the petitioner appeals. II. Analysis A. Statute of Limitations The State claims for the first time on appeal that this court does not have jurisdiction over the petitioner's case because he filed his post-conviction petition outside the one-year statute of limitations provided in Tennessee Code Annotated section 40-30-102 a ; . The State contends that the one-year period for filing the petition began to run on November 4, 2002, when the Tennessee Supreme Court denied permission to appeal. The State also contends that the petition was filed outside the statute of limitations because the circuit court clerk's stamp on the petition shows that it was filed on November 10, 2003. Under Tennessee Code Annotated section 40-30-102 a ; , a petitioner must petition for post-conviction relief within one year of the date of the final action of the highest state appellate court to which an appeal was taken. Pursuant to Tennessee Code Annotated section 40-30-102 b ; , a court does not have jurisdiction to consider a petition for post-conviction relief if it was filed outside the one-year statute of limitations unless 1 ; the claim in the petition is based upon a final ruling of an appellate court establishing a constitutional right that was not recognized as existing at the time of trial, if retrospective application of that right is required; 2 ; the claim in the petition is based upon new scientific evidence establishing that such petitioner is actually innocent of the 3!

DISCUSSION In this work, 20 52 EGFR PTK inhibitor isoflavone analogs 38 dihydroxyisoflavone and trihydroxydeoxybenzoin and 14 dihydroxydeoxybenzoine substituted derivatives ; were able to significantly and dose-dependently inhibit in vitro development of the apicomplexan parasites Sarcocystis neurona, Neospora caninum, and or Cryptosporidium parvum. Five compounds exerted a 95% in vitro inhibition against the three parasites. In an immunosuppressed-gerbil model of cryptosporidiosis, two agents active in vitro decreased oocyst shedding and limited Cryptosporidium parvum intracellular development in the gut and in the biliary tract. Their EGFR PTK inhibitor activities 85% and 87% at a 100-mM dose for RM-6427 and RM-6428, respectively, compared with 97% for genistein [J. F. Rossignol, et al., unpublished data ; are consistent with EGFR PTK being targets for isoflavone analogs. Present results with different coccidia, host cells, and agents suggest common mechanisms of parasite development inhibition, although differential effects of the various analogs tested may also be consistent with different modes of action. Several lines of evidence suggest that isoflavone derivatives act directly on parasites. In the present study, the cell line type did not interfere with the efficacy of agents active against more than one coccidium, and for Cryptosporidium parvum, in vitro activities of RM-6427 and RM-6428 were confirmed in vivo. Dietary supplements containing isoflavones are widely used as alternatives to hormonal therapies for hot flashes, but there is a paucity of data supporting their efficacy. -- OBJECTIVE: To compare the efficacy and safety of 2 dietary supplements derived from red clover with placebo in symptomatic menopausal women. -- DESIGN, SETTING, AND PARTICIPANTS: Randomized, double-blind, placebo-controlled trial of menopausal women, aged 45 to 60 years, who were experiencing at least 35 hot flashes per week. The study was conducted between November 1999 and March 2001 at 3 US medical centers and included women who were recently postmenopausal mean [SD], 3.3 [4.5] years since menopause ; experiencing 8.1 hot flashes per day. Women were excluded if they were vegetarians, consumed soy products more than once per week, or took medications affecting isoflavone absorption. -- INTERVENTION: After a 2-week placebo run-in, 252 participants were randomly assigned to Promensil 82 mg of total isoflavones per day ; , Rimostil 57 mg of total isoflavones per day ; , or an identical placebo, and followed-up for 12 weeks. -- MAIN OUTCOME MEASURE: The primary outcome measure was the change in frequency of hot flashes measured by participant daily diaries. Secondary outcome measures included changes in quality of life and adverse events. -- RESULTS: Of 252 participants, 246 98% ; completed the 12-week protocol. The reductions in mean daily hot flash count at 12 weeks were similar for the Promensil 5.1 ; , Rimostil 5.4 ; , and placebo 5.0 ; groups. In comparison with the placebo group, participants in the Promensil group 41%; 95% confidence interval [CI], 29%-51%; P .03 ; , but not in the Rimostil group 34%; 95% CI, 22%-46%; P .74 ; reduced hot flashes more rapidly. Quality-of-life improvements and adverse events were comparable in the 3 groups. -- CONCLUSION: Although the study provides some evidence for a biological effect of Promensil, neither supplement had a clinically important effect on hot flashes or other symptoms of menopause. Until needed for their roles as antioxidants in plant tissues. A recent study by Setchell et al. 2001 ; reported the pharmacokinetic behavior of naturally occurring isoflavones tested in 19 healthy women12. In this study, time to reach peak plasma concentrations for the aglycones was found to be only about half of the time as for glycones. This finding is consistent with the extra time needed for the enzymatical cleavage of glucose molecules from the glycones to make the aglycones bioavailable.
The natural isoflavone glycosides, daidzin and genistin, were detected from a healthy individual. Both Escherichia coli HGH21 and the gram-positive strain HGH6 converted daidzin and genistin to their respective aglycones, daidzein and genistein. Under anoxic conditions, strain HGH6 further metabolized daidzein and genistein to dihydrodaidzein and dihydrogenistein, respectively17. Eubacterium ramulus, a flavonoid-degrading anaerobic bacterium from the human gastrointestinal tract, was tested for its ability to transform the isoflavonoids including genistein-7-O-glucoside genistin ; , genistein and daidzein. Genistein was completely degraded by E. ramulus via 6'-hydroxy desmethylangolensin to 2- 4hydroxyphenyl ; -propionic acid37. Eubacterium limosum ATCC 8486 ; is a strict anaerobe from the human intestinal tract that is capable of -demethylation of several compounds and was tested for the ability to metabolize three methoxylated isoflavonoids including biochanin A, formononetin and glycitein. The metabolites were identified, using an on line liquid chromatography-electrospray mass spectrometer. E. limosum produced genistein from biochanin A and also produced daidzein from formononetin, after 26 days incubation18. Dietary isoflavones undergo enterohepatic circulation57. When 4-14Cgenistein is infused into the duodenum of rats, it is rapidly absorbed from the intestine, taken up by the liver and excreted into the bile as its 7-O glucuronide conjugate46. Intestinal perfusion studies have indicated that some of the genistein glucuronide produced in the rat small intestine is returned to the lumen46. Intestinal flora appears to be the major source of -glucuronidase in the gut26. Tamura et al. reported that cecal -glucuronidase activities were affected by dietary fiber in both young and aged mice48. It has also been demonstrated that fecal glucuronidase activities were significantly higher in mice fed a diet containing both 5% soy oligosaccharides-0.2% isoflavone SOI diet ; than in those fed a diet containing 5% cellulose-0.2% isoflavone CEI diet ; . Plasma genistein concentrations were significantly higher in the SOI diet group than in the CEI diet group49. Higher floral -glucuronidase activity may lead to efficient re-absorption of the genistein by hydrolyzing the glucuronide form of genistein that is excreted in the bile or is formed in the small intestine and returned to the lumen. Thus, intestinal flora seems to have an important influence on the metabolism and absorption of isoflavones. Studies have shown that only about 3040% of subjects produce significant quantities of equol after isoflavone consumption25, 41. Infants fed soy-containing infant formula in the first 4 months of life when gut microflora is underdeveloped ; can not form large quantities of equol13, 42. Acting together in the PC SPES mixture, individual components may team up to block prostate cancer progression, in part by blocking androgen supported prostate cell growth and by causing cell death. Methods Seven autopsy cases of long-standing PD patients showing prominent peritoneal lesions of HV were selected. A complete medical history was available in each patient. Two patients were diabetic and three had criteria of sclerosing peritonitis. Histological slides from the peritoneum and all the abdominal organs, heart and pericardium, lungs, visceral pleura, and central nervous system were reviewed by two pathologists. All tissue material was formalin-fixed and embedded in paraffin for routine microscopic analysis. When necessary Masson trichromic and periodic acidSchiff stains were used. Grading of HV was performed using a four grade scale 0 to 3 ; Results: Peritoneal lesions were intense in all patients. Prominent HV, fibrosis and variable presence of inflammation, fibrin deposits and calcification were seen. Three patients had pericardial fibrosis and two showed areas of pleural fibrosis in relation with bronchopneumonia. Except for focal lesions of HV in the intestinal submucosa of one diabetic patient HV lesions were limited to the peritoneal membrane. None of the other extraperitoneal tissues, including the fibrotic pericardium and pleura showed such lesions. Conclusions In this study we have demonstrated that extraperitoneal vessels of PD patients show no relevant lesions of HV when compared to peritoneal ones. This observation suggests that PD-related factors are the main contributors to the severity of vasculopathy. Uremia may participate in the development of the lesion but it does not seem responsible of its severity.

In the Phyto-600 group, ERbeta-ir cells represent approximately 52.8% of the total number of cells B ; , whereas TUNEL stained cells account for approximately 33.1% of total cells in AVPV C ; . Independent of diet treatment, most of the TUNEL stained cells express ERbeta.

Decreased activity of either topoisomerases or tyrosine kinases has been implicated in the differentiation of a number of cell types. It is therefore conceivable that genistein, because of its reported ability to inhibit these activities in vitro, may be an inducer of cellular differentiation. We investigated this possibility in human promyelocytic HL-60 and erythroid K-562 leukemia cells and in human SK-MEL-131 melanoma cells. Our results indicated that genistein, in a dose-dependent manner, inhibited cell multiplication and induced cell differentiation. The maturing HL-60 cells acquired granulocytic and monocytic markers. The differentiating K-562 cells stained positively with benzidine, which indicates the production of hemoglobin, an erythroid marker. Following genistein treatment, maturing SK-MEL131 melanoma cells formed dendrite-like structures and exhibited increased tyrosinase activity and melanin content. Experiments were designed to identify the molecular mechanism of genistein's action. Data from our laboratory suggest that this isoflavone triggers the pathway that leads to cellular differentiation by stabilizing protein-linked DNA strand breakage. Other possible mechanisms reported in the literature are discussed.
