Buprenorphine is the only opioid medication available for prescription or dispensation by qualified physicians in medical offices or other settings outside federally approved opioid treatment programs. Buprenorphine is a partial A-agonist and n-antagonist with a unique pharmacological profile that offers a relatively long duration of symptom relief, mild withdrawal, and a low risk of overdose, unless combined with other drugs that decrease respiration, such as benzodiazepines, alcohol, or other sedatives Bickel et al., 1988; Diamant et al., 1998; Fudala et al., 2003; Jasinski, Pevnick, & Griffith, 1978; Kintz, 2002; Lewis, 1985; Mello & Mendelson, 1980; Rance & Dickens, 1978 ; . Although long-term N 30 days ; managed withdrawal using buprenorphine is generally more effective Amass, Bickel, Higgins, & Hughes, 1994; Kakko, Svanborg, Kreek, & Heilig, 2003 ; , there is growing interest and support for shorter term detoxification Amass et al., 2004; Diamant et al., 1998; Lintzeris, 2002; O'Connor et al., 1997; Palmstierna, 2004 ; . For example, shorter term detoxification with buprenorphine may be well-suited for office-based treatment or non-traditional treatment settings, such as community-based residential detoxification centers. Randomized clinical trials comparing buprenorphine and clonidine detoxification have found that buprenorphine clients experience less severe subjective and objective opiate withdrawal symptoms, earlier symptom mitigation, longer treatment retention, and greater detoxification completion success Cheskin, Fudala, & Johnson, 1994; Ling et al., 2005; Lintzeris et al., 2002; Nigam et al., 1993; O'Connor et al., 1997; Oreskovich et al., 2005 ; . In the largest trial to date, conducted at 12 community treatment programs nationwide, the National Institute on Drug Abuse NIDA ; Clinical Trials Network study supported the superiority of buprenorphine to clonidine Ling et al., 2005 ; . The purpose of this project was to examine outcomes of heroin detoxification with buprenorphine versus clonidine in everyday practice. The study design employed a retrospective chart review of 200 heroin-dependent subjects who received detoxification treatment in an acute residential community treatment center either before or after a centerwide switch from clonidine n 100 ; to buprenorphine n 100 ; . The hypothesis was that subjects who received buprenorphine would remain in treatment longer and would be significantly more likely to complete detoxification treatment compared to clonidine controls and coumadin.
