Fter her first, routine bone scan, Darlene Lodico got some surprising news: she had an advanced case of osteoporosis. Yet, Lodico, of Westchester, was only in her 50s, and she felt fine. Her primary care physician referred her to Glenn Sizemore, M.D., an endocrinologist at Loyola University Health System LUHS ; . "After looking at the scan, Dr. Sizemore said to me `I expected a woman in her 80s to walk into my office.' He told me that on a scale of one to 10, I was already a six, " Lodico said, for example, side effects.

Young adolescent women are increasingly at risk for HIV. Twentyfive per cent of sexually active adolescents, male and female, contract an STI each year. The rate of HIV infection is growing faster among adolescent women than in any other group, and in young people more than in any other group heterosexual transmission is the overwhelming cause of HIV infection. Most young women and adolescent girls who contract HIV apparently do not use drugs. They very often have sexual partners who are older than they are, and therefore often have trouble negotiating with them to use condoms. The not fully formed genital tract particularly the immature cervical epithelium ; and the scant vaginal secretions of young adolescent women put them at greater risk of contracting HIV than mature women because they provide less of a barrier to the virus. Increased heterosexual transmission may also be at least partially due to the natural occurrence of cervical ectopy and ectropion. In povertystricken countries, young women are often under pressure to engage in commercial or transactional sex occasionally exchanging sex for material goods or favours ; . Some resort to prostitution to support their families or to pay for their schooling. In some countries, where.

For a small number of patients, even aggressive opiate therapies are not sufficient. Unless alternative pain treatments are found for such patients, they will continue to suffer. For those individuals, their daily lives are often tortuous. As a physician, I acutely aware of the disturbing connection between intractable pain, overwhelming despair, and suicide. I can state confidently, as a physician with an extensive practice and specialized expertise in pain management, that marijuana can prove and has proven ; medically useful to at least some chronic pain patients. Accordingly, I believe that physicians should be able to recommend and or prescribe marijuana to patients for whom it is medically and amiloride.

Caudate neurons were recorded extracellularly with single- or multibarrel micropipettes. Coming WPI, New Haven, CT ; fiber-filled glass capillary tubing was used to assemble pipettes Palmer et al., 1980 ; . Tips of two- or three-barrel micropipettes were broken to diameters of 3 and 4 pm, respectively. The recording barrel was filled with 4 M NaCl or 3 M NaCl and 1 M glutamate to increase neuronal firing rates. All agonist and antagonist effects were observed with or without glutamate. Glutamate activation was not used in the 6-hydroxydopamine experiments. The remaining barrels were filled with drug solutions, which were applied locally by pressure ejection. Neuronal signals were amplified, filtered -3 dB at 300 and 10, 000 Hz ; , and displayed on an oscilloscope. Action potentials were separated from background activity and converted to constant voltage pulses by a window discriminator. To detect action potentials with low amplitude, the window discriminator was adjusted to allow the detection of neuronal activity with a minimal signal-to-noise ratio of 2: l. Ratemeter records of neuronal activity were obtained by connecting the output of the window discriminator, integrated over 2 set intervals, to a strip chart recorder. Caudate neurons were recorded l-2 mm anterior to bregma, 3 mm lateral to midline, and 3-6 mm beneath the brain surface Pellegrino et al., 1979 and amiodarone.

Rates of urinary calcium excretion and levels of markers of bone turnover. In cases of acute symptomatic hypocalcemia, rapid repletion of serum calcium is warranted. An intravenous calcium bolus should be infused in 10 to minutes and repeated as necessary. For a prolonged effect, a continuous infusion of calcium should then be started at a rate of 15 mg of elemental calcium per kg over 4 to 6 hours, which will raise the serum calcium level by 2 to mg dL. This is continued until the patient is on an effective oral regimen of calcium and vitamin D. Commercial preparations of vitamin D and its metabolites are many and include vitamin D2 ergocalciferol ; , vitamin D3 cholecalciferol ; , 1, 25 OH ; 2D calcitriol ; , and synthetic vitamin D dihydrotachysterol ; . Of these, ergocalciferol and calcitriol are most widely available. The dose varies with the disease, its etiology and severity, and the form of vitamin D used. Ergocalciferol Calciferol; Drisdol ; is the cheapest form of vitamin D and can be given orally or intramuscularly. Severe malabsorption associated with gastrointestinal, liver, or biliary disease requires higher doses and intramuscular injection for adequate repletion. A disadvantage of ergocalciferol is that it may take as much as 1 month for its effect to be seen. Calcitriol Rocaltrol ; has a faster onset of action 46 hours ; and is most useful when rapid correction of vitamin D is required as in our case ; and in chronic renal failure with decreased 1-alpha-hydroxylase ; . It can be given orally or intravenously. Of importance: patients taking vitamin D supplements need to take at least 1 to 1.5 g of supplemental calcium a day. s HOSPITAL COURSE The patient receives a bolus of intravenous calcium and a continuous calcium infusion for her symptomatic hypocalcemia. She is also started on intravenous calcitriol and vitamin B12. Her pain, spasms, and anxiety improve considerably over the next 2 to 3 days. She is discharged home taking oral ergocalciferol, a multivitamin that contains trace elements, and high oral doses of cobalamin and calcium.

FIN. 8. Mechanism of action of crlciferol in uiuo and the polyene antibiotic filipin, in &to, on intestinal calcium transport. The lop panels indicate schematically the effects of filipin F ; in intestinal tissue from calciferol + D ; -treated chicks; the bottom panels indicate the effects of filipin in the intestinal tissue from rachitic -D ; chicks. step appears to be dependent on oxidative metabolism in the rat 5, 18 ; , but not in the chick 6, 52 ; . Shown in Fig. 8 is a schematic diagram showing the possible molecular basis of calciferol and filipin action on the ileal mucosal membrane. Adams et al. 22 ; suggested that filipin is apparently able to effect a structural alteration or reorganization of the mucosal brush border membrane of the -Da chick so that a previously inactive calcium transport system became a ; an active transport process, 5 ; specific for calcium, c ; cold sensitive, and sensitive to N-ethylmaleimide 22 ; . All of the above mentioned characteristics have also been found to be properties of the + Dz transport system. However, an important consequence of the present communication is the finding that filipin, in vitro, can also enhance the uptake of calcium in the + Da ileum but that this is not reflected in an over-all change in the rate of transport, J . This clearly suggests that the rate-limiting step for the transport process is not at the brush border membrane. In addition, the filipin and calciferol effects were found to be additive, thus confirming the hypothesis that calciferol does mediate an effect at the mucosal membrane surface. Thus the rate-limiting step of over-all transport of calcium could either be intracellular or at the basal-lateral membrane of the epithelial cell. Wong and Norman * have reported that a ouabain-sensitive sodium and potassium ion-activated Na + -K + ; -ATPase located at the lateral-basal membrane of the epithelial cell is involved in the extrusion of calcium. This active pump was further shown to be independent of calciferol status of the animal. Therefore, the active transport component in the filipin-treated, rachitic chick ileum could be due to an increase in intracellular calcium concentrations promoted by an increased translocation of calcium across the mucosal brush border membrane. It can also be suggested that this uptake step may be the rate-limiting step in the over-all transport of calcium Fig. 1 ; . Therefore, if this calcium pump can limit the maximal amount of calcium which can be extruded at a given time, this might explain why filipin did not mediate an increase of over-all transport but did enhance the uptake of calcium. The conclusion reached when all these observations are taken collectively is that calcium moves acrossthe brush border mem8 Submitted for publication.

Increased life expectancy from lowered mortality rate and a decline in the fertility rate have led to an aging global population. The rate of aging is accelerating especially in developing countries which, ironically, lack adequate resources to cope with the consequences. The socio-economic impact of population aging is enormous when, as the result, labour supply decreases, national savings drop, dependency ratio rises and demand on healthcare and social support escalates. Apart from issues such as housing, communications and transport, age-related medical problems are considerable. These may be gender-specific conditions or general problems regardless of sex.

Birbilis H, Kalamatas T, Frigana N, Kapetanakou K, Savvakis G Internal Medicine Department, Livadia General Hospital, Greece We present a rare case of severe vitamin D deficiency that had remained undiagnosed and untreated for a long period of time. A 27 year-old white male who was bedridden and mentally retarded, was hospitalized in our clinic with respiratory distress. His had a significant past medical history of lifelong seizures. The patient had been on anticonvulsant treatment since age 3. Since age 14, he had developed worsening contractions and had been essentially bedridden since age 22. On physical examination he had extensive scoliosis of the thoracic and lumbar spine, with saddle deformities of his lower extremities. His radiologic workup, in addition to the scoliosis, showed diffuse osteopenia and decreased height of most vertebral bodies. His laboratory workup showed serum calcium Ca ; 5.1mg dL 8.5-10.1 ; , phosphorus 2.5mg dL 2.5-4.9 ; , total protein 6.4g dL, albumin 2g dL, urea 14mg dL, creatinine 0.4mg dL, alkaline phosphatase LP ; 689U L 50-136 ; , urinary Ca 24h 20.8mg dL 100-300 ; , parathyroid hormone PTH ; 282pg mL 1565 ; , 25- OH ; D2 0.7ng mL 0.5-4.8 ; , 1, 25- OH ; 2D3 3.0ng mL 5-41.8 ; and total vitamin D 3.7ng mL 6.346.4 ; . The patient was started on Ergocalciferol 20, 000 IU Q 7D and Ca 2, 000mg QD. After 10 months he was more mobile, with increased muscular strength. His laboratory follow up workup showed an increase of the Ca level to 8.5mg dL and a decrease of both the ALP to 359U L and the PTH to 93pg mL. Vitamin D.
